Medical Records provides support to all patient care areas within the hospital and its satellite facilities by providing and maintaining patient medical record information. This support is provided with professionalism, technical expertise and attention to detail.

Medical Records is open Monday through Friday from 8 a.m. to 5 p.m. Completed authorization forms can be faxed to (480) 728-9618.
